Built 1919 by Rapp & Rapp (auditorium by G. Albert Lansburgh) for the Orpheum Circuit. Opened 17 March 1919 as a vaudeville theatre and cinema. Later renamed "RKO State-Lake Theatre". 1941 converted into a cinema. Closed 28 June 1984. Auditorium demolished and converted into TV studios and offices.
